Service Coordinator
Community Name: Capitol Station. The Service Coordinator is responsible for coordinating programs and services to assist clients or residents in maintaining the highest practicable level of independence and physical, mental, and psychosocial well-being.
Essential Functions
- Assists and educates clients or residents and families to acquire and utilize community services to maintain a self-reliant lifestyle.
- Acts as a liaison between community agencies, service providers and clients or residents; seeks out new services and identifies low-cost providers and/or negotiates discounts.
- Monitors the delivery of services to clients or residents to ensure they are appropriate, timely and satisfactory.
- Maintains a resource directory of available community services.
- Maintains professional relationships with all clients or residents in the housing community to identify areas of need and make referrals to community agencies as necessary.
- Maintains resident files, including intake information, assessment, service referral and termination information, quarterly review and follow‑up, human or civil rights violations, and resident, family and provider meeting or communication notes.
- Conducts quarterly quality service reviews and follow‑up.
- Submits all required reporting data within specified timeframes.
- Networks with service providers to maintain necessary relationships and pursues avenues for additional services through private, local, state and federal sources.
- Might require some travel for conferences, trainings, and other company events.
- Performs all other duties as assigned or directed.
UCH Engage Housing Service Coordinator
- Promotes wellness activities for all residents, including setting up onsite or mobile health care services and screenings, encouraging residents to be proactive in meeting their own social, psychological, and physical needs.
- Coordinates educational events for residents, families, and staff on available community resources, including eligibility requirements and referral processes.
- Assists residents in building informal support networks with other residents, family members and friends.
- Coordinates volunteer support programs with service organizations in the community.
NaviGuide Community Service Coordinator
- Assesses client needs to establish individual service plans.
- Acts as a family support system and provides community resources for caregivers.
- Provides home visits during inclement weather.
- Maintains client, family and provider meeting and communication notes.
- Commutes daily for client home visits.
- Requires daily travel to client homes and community service providers; attends trainings, conferences, and other company events.

Qualifications
- Education: Bachelor’s Degree in social work (preferred).
- Experience: Two (2) or more years’ experience working with social service delivery to elderly populations; working knowledge of geographic resources available to serve the community.
- Computer Skills: Proficient in operating a computer, the Internet, Microsoft Office, and other relevant software; basic office equipment.
- Certifications & Licenses: Membership and familiarity with the American Association of Service Coordinators (helpful).
- Other Requirements: Proven experience in service management or facilitation; strong interpersonal and communication skills; ability to read, write, understand, and speak English; work closely with residents, family members, legal representatives, and housing community staff; function independently; high flexibility and personal integrity; comply with all terms and conditions of the United Church Homes Employee Handbook.
