Job summary

Paycom - ATS seeks an Early Intervention Service Coordinator in South Carolina to support infants and toddlers through BabyNet processes. You will build trusted relationships with families, develop IFSP goals with a multidisciplinary team, and ensure timely service delivery.

This role requires traveling within the territory, documenting services, and guiding families to healthcare and community resources while upholding privacy and policy standards.

Qualifications

  • Bachelor's degree in Education, Early Childhood, Special Education, or Elementary Education.
  • At least one year of experience with infants/toddlers or birth to 5 with disabilities.
  • Submit application to TECS and meet SC Infant-Toddler Credential requirements.
  • Willingness to travel territory as needed.
  • Experience working with an interdisciplinary team.

Responsibilities

  • Maintain a minimum caseload of 35 clients/families (FT).
  • Establish professional relationships with families based on trust and respect.
  • Functions as part of the child's interdisciplinary team and consult with professionals.
  • Develop IFSP goals/outcomes with families and IFSP monitoring.
  • Ensure procedures/timelines per IDEA and BabyNet are followed.
  • Provide information on local community, state, and national resources.
  • Participate in interagency meetings and coordinate with outside agencies.
  • Assist families in accessing health care, resources, and funding sources.
  • Serve as transition coordinator for referrals and follow-up with Part B agencies.
  • Coordinate transition to Part B services with school districts.
  • Provide families with information on current research and evidence-based practices.
  • Offer linkage, referral, advocacy, and crisis support as needed.
  • Assist families in advocating for their child.
  • Submit accurate and timely routine documentation (clinical service notes).
  • Maintain confidential, organized, up-to-date client records.
  • Attend staff meetings and trainings as required; travel territory as needed.
  • Performs other duties designated by supervisor or Executive Director.

Job description

Essential Job Functions:Maintains a minimum caseload of thirty-five (35) clients/families (if full-time).Establishes and maintains professional relationships with families based on trust and respect as support and assistance is provided.Functions as a member of the child’s interdisciplinary team, shares information, and consults with other professionals.Facilitates the early intervention process and works with families, caregivers, and other IFSP team members to develop functional Individualized Family Service plan (IFSP) goals or outcomes.Monitors and evaluates the IFSP for each family, ensuring that appropriate procedures and timelines are followed as established by policies and procedures established by Part C of IDEA and BabyNet.Supports each client/family by consistently sharing relevant information about local community, state, and national resources.Participates in interagency meetings, telephone calls and/or face to face contact with other IFSP team members or outside agencies.Assists the family in gaining access to health care, community resources, services, and/or funding sources to help meet their needs.Serves as transition coordinator for the family by submitting transition referrals, maintaining timelines, and providing follow-up services with receiving Part B agencies.Works closely with the local school district and other Part B agencies to coordinate the child’s transition to Part B services.Provides families with information on current research and evidence-based practices related to the needs of their child.Provides linkage, referral, advocacy, service planning, and crisis support as needed.Assists families in establishing their role as an advocate for their child.Submits accurate and timely routine documentation (clinical service notes) as required by DHHS and KIDS policies and procedures.Ensures client records are kept confidential, accurately maintained, organized, and up to date as required by DHHS and KIDS policies and procedures.Attends all KIDS staff meetings and/or other meetings and trainings as required by the Executive Director or EI Program Manager.Must be willing to travel established territory as needed.Performs other duties as designated by the Service Coordination Supervisor, EI Program Manager, or Executive Director.MINIMUM QUALIFICATIONSA Bachelor’s degree in one of the following areas:Education: Early Childhood, Special Education, Early Childhood Special Education, or Elementary EducationChild/Human DevelopmentInterdisciplinary Studies (with approval from BabyNet State Office): with a focus in Early Childhood, Special Education, Early Childhood Special Education, or Elementary EducationChild LifeFamily Studies/Family and Consumer SciencesLicensed Professional CounselorLicensed Marriage and Family TherapistPsychologyPublic HealthSocial WorkSociologyAnd (REQUIRED): At least one year of documented experience working with infants and toddlers OR working with children ages birth to 5 years with disabilities.Must submit an application to the Team for Early Childhood Solutions (TECS) office and meet requirements for the South Carolina Infant-Toddler Credential.PREFERRED: At least one year of experience with South Carolina's BabyNet Early Intervention Program.
