Job summary

EmpowerMe Wellness is seeking a licensed therapy professional to lead therapy and wellness services for older adults within our EmpowerMe 360 program. You will work as part of a collaborative team to assess residents, develop personalized care plans, and deliver evidence-based interventions across modalities, including PT, OT, and SLT.

Qualifications

• Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP).
• Strong collaborative spirit and teamwork.
• Passion for empowering seniors to live their best lives.

Responsibilities

• Provide therapy and wellness services for older adults.
• Conduct assessments and create personalized care plans with evidence-based interventions.
• Support care coordination with wellness check-ins, caregiver communication, and telehealth support.
• Maintain professional and ethical standards aligned with company initiatives.
• Contribute to program development and continuous quality improvement.
• Help residents maximize independence, confidence, and satisfaction.
