Job summary

EmpowerMe Wellness is seeking qualified therapists to support older adults within our EmpowerMe 360 program. You’ll deliver therapy across physical, occupational, or speech disciplines, conduct assessments, and develop personalized care plans as part of a collaborative team.

You will engage in care coordination, wellness check-ins, and telehealth support while upholding professional and ethical standards and driving program development for resident independence and safety.

Qualifications

• Current professional therapy license in the applicable discipline.
• Passion for empowering seniors to live their best lives.
• Strong clinical skills and collaboration with a care team.

Responsibilities

• Provide therapy and wellness services for older adults
• Conduct assessments, create care plans, and deliver interventions
