Waltonwood is seeking a compassionate Wellness Nurse to join our Resident Care team. You’ll support assessments, medication management, daily rounds and staff education to maintain residents’ health and quality of life.

Ideal candidates have a current LPN license, CPR certification, and at least 1 year of nursing experience, preferably in assisted living or memory care. This on-site role reports to the Executive Director and contributes to regulatory compliance and teamwork.

Qualifikationen

• Licensed Practical Nurse license required in the state of application.
• CPR certification required.
• 1+ year nursing experience; memory care or assisted living preferred.
• Experience in assisted living or memory care preferred.

Aufgaben

• Oversee quality assurance by monitoring assessments, referrals, care plans and coordination.
• Provide wellness care and follow up on resident concerns.
• Administer and assist with medication management to ensure correct dosing.
• Assess vitals and report changes to the doctor for ongoing health.
• Assist with care conferences, doctor days, and care plan creation.
• Educate associates on health topics and participate in in-service training.
• Maintain regulatory compliance for licensed communities.
• Contribute to on-call rotation and assist with other duties as assigned.
