Job summary

The Residence at Boyertown is seeking a compassionate Resident Wellness Assistant to provide hands-on daily living support, mobility assistance, and personal care to residents.

You will work with the Wellness Director and care team to implement individualized care plans, maintain a safe environment, and communicate clearly with residents and families while upholding dignity and ethics.

Qualifications

• High School Diploma or GED required.
• Home Health Care, CNA, CPR certification preferred.
• Compassionate, patient-centered approach.
• Strong communication and teamwork.
• Time management and attention to detail.
• Proficient in Microsoft Office and EHR.
• Clear written and verbal English communication.
• Flexible hours including weekends/holidays.

Responsibilities

• Assist residents with daily living activities and personal care.
• Ambulate residents to activities, dining, or other locations.
• Assist with personal hygiene tasks.
• Lift residents as needed.
• Communicate with residents, families, and staff while maintaining confidentiality.
• Collaborate with Wellness Director and care team on individualized care plans.
• Maintain a clean, safe medication area.
• Provide compassionate service respecting mobility, cognition, and sensory needs.
• Build relationships with residents and team while upholding ethics.
