Job summary

Blakeford Senior Life seeks an experienced care manager to serve LiveWell members, acting as navigator and advocate for aging in place. You will champion healthy aging practices and support members toward independence at home.

You will conduct regular wellness screenings, coordinate home care services, and analyze health data to help members meet their goals while collaborating across Blakeford service lines.

Qualifications

• Bachelor's degree in nursing, social work, occupational therapy, or related wellness discipline is required.
• 5+ years of experience working with older adults in geriatrics, wellness, rehabilitation, care coordination, or community-based services.

Responsibilities

• Serve as a care manager, navigator, and advocate for LiveWell members.
• Champion healthy aging practices to improve independence and home living goals.
• Conduct regular health and wellness screenings and help members set improvement goals.
• Coordinate, plan, and oversee member care and services at home.
• Collect, track, and analyze health data to support member goals and independence.
• Coordinate wellness activities, resources, vendors, and interventions for members.
• Collaborate across Blakeford service lines to support growth and outreach.
• Other duties as assigned.
