Job summary

St. Gabriel's Community is seeking a Wellness Assistant to enhance residents' quality of life through wellness activities and programs in the community.

You will help design and run programming, support assessment processes through data collection, and assist in implementing care plans tailored to resident needs. The role requires a high school diploma or equivalent, with preference for long-term care experience and relevant certifications (BLS and First Aid).

Qualifications

• High School Diploma or equivalent is required.
• Experience in long term care is preferred.
• BLS Certification required.
• First Aid Certification required.
• If leading the program, qualifications must meet federal regulation standards.

Responsibilities

• Assists in developing the community’s wellness programming.
• Facilitates daily events and programming to meet residents’ needs.
• Performs data collection to assist with residents’ assessments.
• Assists in implementing the plan of care based on residents’ needs.
