Job summary

S:US is seeking a Senior Service Coordinator to lead a team within the SSVF program on Long Island, managing a robust caseload of veterans and their families and ensuring housing-first service delivery. You will supervise up to four SSVF Service Coordinators, conduct comprehensive assessments, and coordinate rapid rehousing and homeless prevention services with community partners.

The role requires a bachelor’s degree and ability to perform fieldwork across the service area, with a strong focus

Qualifications

• Bachelor’s degree is required.
• Ability to complete fieldwork across the S:US service area is required.

Responsibilities

• Manage a caseload of SSVF program participants, including those with high barriers to housing stability.
• Directly supervise up to four SSVF Service Coordinators/field staff.
• Provide short-term intensive case management to veteran families to achieve housing stability.
• Develop and implement individualized housing stability plans with veterans/families to address barriers.
• Monitor and evaluate veteran participants’ progress and adjust housing plan as needed.
• Complete documentation to ensure compliance with funding requirements and agency standards.
• Coordinate Rapid Rehousing and Homelessness Prevention services with participants and social service team.
• Lead staff meetings and participate in SSVF and Veterans Services team meetings.
