Qualifications

• Bachelor’s Degree in Social Work or Human Service field, and two years direct care experience in Human Services preferred; or equivalent
• Valid Driver’s License and acceptable driving record
• Travel is required; use of personal vehicle may be necessary

Responsibilities

• Conducts assessments to accept, deny, and refer applicants.
• Verifies and documents eligibility and maintains client service records.
• Assists individuals in locating, securing and retaining suitable housing.
• Develops a housing stability plan with the client and coordinates services.
• Supports clients in increasing financial security and accessing benefits.
• Provides crisis response services and trauma-informed care as needed.
• Collaborates with landlords and referral sources to meet client needs.
• Ensures safety and accuracy in reports and statistics; adheres to guidelines.
