Job summary

Apex Home Health, Hospice & Home Care is seeking a Transplant ICU Clinical Nurse Coordinator to lead and coordinate complex care for adult transplant patients in a high-acuity ICU, providing advanced clinical assessment and managing therapies.

You will collaborate with surgeons, intensivists, and the interdisciplinary team, oversee medication/infusion management, educate patients and families, and contribute to quality improvement in a compassionate, team-based setting.
