Elsevier is expanding Wellsheet into a new clinical AI experience and is seeking engineers to own end-to-end agent features from backend to UI. You will work closely with a senior platform team to ship AI-driven clinical tools that clinicians use at the bedside across Epic, Cerner, and Athena.
You will contribute from day one to design, implement, and evaluate features, with a focus on measurable outcomes and robust tooling.
Wellsheet is a clinical decision support platform used by more than 20,000 providers across 100+ hospitals. We embed directly in the EHR (Epic, Cerner, Athena) and put AI-generated clinical context in front of clinicians at the bedside. In 2026 Wellsheet became part of Elsevier's Clinical Solutions group, and we're now building the next generation of our product: a new clinical AI experience shipping to our first hospital clients in early 2027, backed by an integration platform we've run in production for years. We're a small, all-senior engineering team. You'll own real surface area from your first weeks, work directly with the people who built the system, and ship software that clinicians use during patient care. No healthcare background is required. We train the clinical and interoperability domain; we hire for engineering judgment.
The Platform squad runs the machinery underneath everything clinicians see: real-time EHR sync across Epic, Cerner, and Athena, FHIR ingestion for 100+ hospitals, and vocabulary normalization across 125+ clinical terminologies. The squad is led directly by the technical leader behind Wellsheet's platform. Its next major build, which you'll help kick off, is platform API that other Elsevier clinical products and internal teams will consume. The squad also owns the internal tooling for managing the platform and the health of the stack itself, from framework upgrades to developer tooling.
Owning agent-powered features end to end: prompt and orchestration logic, backend plumbing, and the UI where the output lands.
Taking loosely specified feature ideas ("clinicians want the chart review agent to explain itself") through design, implementation, and measured rollout.
Building and extending eval coverage for the features you ship; nothing ships unmeasured.
Building new ways for agents to present their work, from richer in-chat results to interactive views.
Improving the synthetic data simulator and the tooling around it.
Work across squad boundaries with Product to land agent features in the clinical experience. Pairing with the engineer leading our agent work, ship an improvement to a production agent feature end to end, and add the eval coverage that proves it. By month three you'll own an agent surface outright.
Possess current Software Engineering development skills with TypeScript and Ruby experience; you treat prompts and models as components in a system you're responsible for. End-to-end ownership from backend to UI, with the judgment to keep the seams clean.
Instinct to measure: you'd rather have a small honest eval than a big impressive demo.
Comfort with ambiguity; much of this work has no established playbook yet.
AI-assisted development as a default: coding agents and spec-driven development are already part of how you work.

U.S. National Base Pay Range: $95,300 - $158,800. Geographic differentials may apply in some locations to better reflect local market rates. If performed in New York, the base pay range is $104,800 - $174,700. If performed in New York City, the base pay range is $114,300 - $190,500. If performed in Rochester, NY, the base pay range is $95,300 - $158,800. This job is eligible for an annual incentive bonus.
