Role

Senior Software Engineer responsible for building and maintaining Lithos’s core clinical telehealth platform and partner-facing API surfaces. You will work across backend, frontend, and integrations to deliver reliable, auditable clinical workflows and high-quality partner developer experiences.

Key Responsibilities

• Develop and maintain the core clinical platform (Ruby on Rails, PostgreSQL) modeling clinical workflows as explicit state machines with database-enforced invariants.
• Implement and evolve the partner-facing public API using a contract-first approach (OpenAPI) with typed clients and additive versioning.
• Build clinician and operations consoles using Next.js and TypeScript, collaborating on a shared design system.
• Implement and maintain integrations (e-prescribing, pharmacy fulfillment, lab networks, document generation) and ensure reliable webhook semantics.
• Operate and improve cloud infrastructure on GCP, CI/CD pipelines, environment-per-stage deploys, and human-gated production releases.
• Own features end-to-end: design, implementation, tests, docs, rollout, and production verification.

Requirements

• 6+ years building production backend systems.
• Deep comfort with Ruby on Rails and PostgreSQL, including transactional thinking, constraints, and idempotency.
• Strong TypeScript and React skills; ability to carry features across the full stack.
• Experience building systems that are correct under failure: retries, race conditions, exactly-once side effects, and state machines.
• Clear written communication for design notes, code comments, and reviews.
• Comfort using modern AI-assisted development tools in daily workflows.

Nice to have

• Experience in healthcare or other regulated domains (HIPAA, clinical workflows, lab or pharmacy integrations) or fintech-level rigor.
• Experience running workloads on GCP and debugging production from logs.
