Qualifications

• Master's Degree in Social Work required.
• Permanent LCSW licensure required.
• 5 years of experience in child welfare, mental health/addictions, health, school, or community/organizational settings.
• Crisis Prevention Intervention certification within 6 months of hire.
• Knowledge of psychosocial assessment and crisis intervention for patients and families with common mental health and substance use disorders.
• Ability to coordinate complex discharge planning to ensure safe discharge to the next level of care.
• Knowledge of state and county adult and child protective legal mandates.
• Strong collaboration with hospital administrators, medical teams, law enforcement, APS/DCS, schools, probation, and other community agencies as indicated.

Position Title: Social Worker-MSW LCSW Senior

Position Summary

The Social Worker - MSW LCSW Senior provides advanced clinical social work services to patients and families at Fishers Hospital, with a focus on complex discharge planning, crisis intervention, and mental health assessment. In addition to direct patient care, this senior-level role provides clinical supervision to social workers pursuing LCSW licensure, serves as a field instructor for social work students, and acts as a subject-matter resource across the care team on child and adult protective matters, guardianship, and end-of-life care coordination. The role requires close collaboration with hospital administrators, medical teams, and community agencies to ensure patients receive safe, timely, and well-coordinated care.

Key Responsibilities

• Meets face-to-face with patients and families.
• Acts as a preceptor to facilitate learning and develop educational materials for team members, patients, and families.
• Provides clinical supervision for social workers who are working toward their LCSW licensure.
• Serves as a field instructor for social work students.
• Builds strong working relationships with hospital administrators and medical teams to drive quality and process improvement.
• Serves as a resource and role model for students, interns, LBSWs, and LSWs.
• Participates with the clinical team in developing treatment plans to resolve patients' psychosocial issues and meet psychosocial needs, including consulting with other providers on mental health diagnosis.
• Communicates and collaborates with all members of the health care team to facilitate the episode of care, ensuring patients receive safe, timely services and that patients and families are provided all necessary resources and education.
• Interprets and communicates complex patient/family needs and role models caring practices to other caregivers.
• Assesses patients' and families' psychosocial risk factors, including prior functioning levels, adequacy of support systems, reaction to illness, and ability to cope.
• Counsels patients and families on the emotional, social, and financial consequences of illness and/or disability, and accesses and mobilizes family/community resources to meet identified needs.
• Provides mental health assessments of patients upon physician referral.
• Provides therapeutic intervention to facilitate complex discharge planning.
• Serves as the point person for child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence cases.
• Serves as liaison between Indiana University Health and state and county child and adult protective teams.
• Maintains principles and practices of crisis intervention and de-escalation techniques.
• Provides intervention in guardianship (temporary/permanent), foster care, adoption, and mental health level II placements.
• Provides specialized mental health services, including crisis intervention and addiction assessments, with recommendations for psychiatric referral or community resources.
• Assists with dangerous and potentially dangerous situations, taking a leadership stance to provide clinical intervention.
• Supports patients undergoing acute crisis, using de-escalation techniques as needed to avoid episodes of physical violence.
• Utilizes effective interviewing principles with alleged victims, perpetrators, family members, and significant others.
• Provides intervention for end-of-life care, guardianship (temporary/permanent), foster care, adoption, and related needs.

Qualifications

• 5 years of experience required in one of the following areas: child welfare, mental health and addictions, health, school, or community/organizational.
• Certification in Crisis Prevention Intervention or an alternative de-escalation training required within 6 months of hire.
• Knowledge of evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention for patients and families with common mental health and substance use disorders.
• Knowledge of disease processes and human behavior as they relate to health-related social needs.
• Complex discharge planning skills to ensure safe discharge to the next level of care.
• Knowledge of state and county adult and child protective legal mandates.
• Knowledge of the principles of growth and development across the life span, including care at end of life.
• Ability to identify decision makers and coordinate and execute goals-of-care conversations to provide excellent care and manage throughput.
• Experience adapting services to geographically, culturally, and financially diverse populations.
• Ability to develop and maintain strong working relationships with hospital administrators, medical teams, law enforcement, APS/DCS, schools, probation, and other community agencies as indicated.
• Computer literacy and proficiency documenting in an electronic medical record.
• May be scheduled to work on-call and holiday rotation.
