Job summary

East Side House Settlement seeks a Lead Social Worker in the Bronx to assess residents, coordinate care, respond to crises, and advocate for housing stability and well-being. You will develop service plans, collaborate with Case Managers, and link clients to community resources at the North Cove/River Avenue site.

The ideal candidate holds an MSW and has experience with homelessness, trauma-informed practices, and strong documentation and engagement skills.

Qualifications

• MSW from an accredited institution.
• Experience with homelessness, housing instability, or other complex social needs.
• Knowledge of trauma-informed, strengths-based, person-centered practices.
• Experience with needs assessment, crisis intervention, care coordination, and referrals.
• Knowledge of NYC social services, behavioral health resources, and public benefits.

Responsibilities

• Serve as the primary social work resource at the site, supporting residents with complex needs and guiding the team.
• Conduct comprehensive assessments for residents with barriers, crisis situations, safety concerns, or needs requiring professional judgment.
• Provide focused case support and interventions for higher-acuity cases, including crisis response, safety planning, advocacy, and care coordination.
• Develop individualized service plans with residents and monitor progress toward housing stability and family well-being.
• Provide case consultation to Case Managers and coordinate referrals, follow-up, and escalation.
• Connect residents to behavioral health, healthcare, benefits, housing, legal services, education, workforce development, childcare, and other resources.
• Coordinate with external providers for specialized or ongoing services and warm handoffs.
• Review case documentation to ensure timely, accurate, and confidential records.
• Participate in case conferences, staff meetings, and program planning.
