Job summary

U.S. Department of Veterans Affairs in Augusta, ME seeks a Senior Social Worker (Hospice and Palliative Care) to provide comprehensive psychosocial support for Veterans with advanced illnesses.

The role focuses on end-of-life care, coordination with the VA Maine Hospice and Palliative Care teams, and education for Fellows and trainees. The position requires licensure, CSWE-accredited MSW, and at least two years of post-licensure experience in hospice/palliative settings.

Qualifications

• Master's degree in Social Work from CSWE-accredited program required.
• Licensed or certified at the master level to independently practice social work per state requirements.
• Two years post-licensure experience in advanced social work practice, with hospice/palliative focus preferred.

Responsibilities

• Provide social work services to Veterans on inpatient Hospice and outpatient Palliative Care.
• Assess patient understanding of diagnosis and treatment options.
• Support end-of-life planning and advance directives with Veterans and families.
• Coordinate with interdisciplinary teams to facilitate goals of care discussions.
• Assist with discharge planning and placement in appropriate care settings.
• Educate staff and trainees on psychosocial aspects of palliative care.
