The Social Worker is dedicated to helping individuals, families, and communities navigate life's challenges by providing support, resources, and advocacy. This role involves assessing patient needs, developing personalized plans, offering counseling, and collaborating with other professionals to ensure comprehensive care. Social Workers may work across a variety of settings, with a focus on empowering patients and advocating for their well-being. Social Workers provide direct patient care under the guidance of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of the organization.
