Responsibilities

• Lead multidisciplinary team meetings and ongoing treatment planning.
• Supervise and mentor junior social workers and interns.
• Coordinate care with facility staff, families and external agencies for continuity of services.
• Monitor progress, adjust interventions, and ensure compliance with regulations and policies.
