Job summary

Northwell Health in New York seeks an experienced Social Worker to lead, guide, and coordinate staff in providing social work services across the continuum of care.

The role focuses on assessing psychosocial needs, developing health and welfare resources, and collaborating with the care team to support patients and families. A NY clinical social worker license and 3–5 years of relevant experience are required.

Qualifications

• Master of Social Work required.
• Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.
• 3-5 years of relevant experience, required.

• Assists in the formulation and development of departmental and interdepartmental programs, policies and procedures.
• Contributes to the assessment of social health needs in the facility and community; participates in the development of effective health and welfare services/resources.

• Participates in appropriate research, staff development programs, and community liaison activities.
• Contributes to educational activities including presentation, training of graduate students, attending/championing workshops and seminars.
• May participate in hospital committees to discuss social work practice and procedures.
