Responsibilities
  • Valid license as an LCSW issued by the NYSED and one year of supervisory experience utilizing casework, group work and/or community organization methodologies.
  • Screen charts for historical and clinical information, conduct psychosocial evaluations through interviews with patients and/or identified surrogates, and formulate appropriate social work treatment plans.
  • Work within health care setting offices or in community settings within NYC to provide individual and group counseling, as well as concrete social services.
  • Consult with professionals in other disciplines to determine and implement a multidisciplinary approach to patient care, develop treatment plans, evaluate patient progress, develop discharge and aftercare plans, and recommend alternative actions or treatment.
  • Provide outreach services, make referrals to other governmental, public and/or community agencies, prepare patients to accept services, provide direct social work services on high‑risk cases, and escort patients as needed.
  • Act as a mandated reporter, documenting and reporting suspected cases of child abuse or neglect to the New York State Central Registry.
  • Appear and testify at administrative and court hearings, as necessary.
  • Assist in plans for institutional or hospital discharge and other post‑institutional/hospital care; evaluate alternate care arrangements where needed.
  • Participate in staff conferences and related social work meetings, special studies, committees, and research projects, and internal/external quality assurance/performance improvement activities, programs, and trainings.
  • Prepare reports and related paperwork; keep appropriate records.
  • Ensure ongoing compliance with NYC Health + Hospitals policies and procedures, national standards, and other applicable external regulatory requirements, guidelines, and accreditation authorities.
  • May assist in the provision of clinical services only under supervision of a New York State LCSW, licensed psychologist, or psychiatrist.
  • Perform other related duties as directed and with required supervision.
Supervision and Leadership Tasks
  • Mentor and assist less experienced social work staff in social work techniques and serve as a resource in various aspects of advanced social work practice.
  • Supervise graduate and undergraduate social work students, volunteers and other auxiliary personnel as needed.
  • Provide or supervise clinical services under or independently as an LCSW.
  • Provide direct clinical supervision, screen and assign cases, schedule staff, review staff work, evaluate performance, and maintain time and leave records.
  • Provide and document clinical supervision of Licensed Master Social Workers toward advanced Social Work licensure, in accordance with NYS licensure laws.
  • Maintain individualized supervision logs and ensure timely completion of attestation forms required by NYSED Office of the Professions.
  • Conduct or lead special studies and projects, including data collection and evaluation related to vulnerable patient populations, and prepare and present reports.
  • Establish and maintain professional social work standards concerning interview techniques, assessment, treatment planning, and documentation.
  • Interpret clinical information, prepare and submit comprehensive reports, and recommend recommended courses of action.
  • Orient and educate members of other professional disciplines by developing training curricula and presenting on social work concepts and functions.
  • Train and instruct staff in agency policies, procedures, and social work techniques.
  • Ensure compliance with accepted social work practices, principles, and operational adherence to government rules and reporting requirements.
  • Participate in conferences, committees, and meetings on‑and off‑site with supervisory/managerial staff, other clinical professionals, and representatives of government and other agencies.
  • Analyze population health issues, serve as liaison to community agencies, facilitate communication, and utilize available community resources.
  • In the temporary absence of the supervisor, perform the duties of that position.
General Duties
  • Provide clinical supervision and assign work to and provide professional expertise and supervision to a number of social work units on complex cases.
  • Coordinate the work of staff with that of other government and non‑governmental agencies to facilitate effective working relationships and enhance patient experience.
  • Plan, develop, implement and coordinate programs and activities to attain stated objectives and meet identified needs.
  • Consult and provide technical assistance to subordinate staff and others by researching and analyzing information and recommending corrective actions.
  • Assess staff training needs, develop and revise lesson plans, and conduct training sessions, workshops or conferences.
  • Interpret and communicate procedures, regulations and program directives.
  • Respond to mail, telephone, and in‑person inquiries; obtain needed information to assist patients, agencies, courts and others.
  • Participate in drafting policy and procedural manuals, memos or other instructional guides for Central Office approval.
