Veterans Health Care System of the Ozarks is seeking a Senior Social Worker assigned to Home Based Primary Care (HBPC). The position involves delivering direct services to adult and geriatric Veterans and their families within a 50-mile radius of the HBPC clinic.
Responsibilities include psychosocial assessments, treatment planning with other disciplines, discharge planning, counseling, and education on VA resources.
This Senior Social Worker position serves the Veterans Health Care System of the Ozarks, Social Work Service and is assigned to Home Based Primary Care (HBPC). The locations for services can include a 50-mile radius of the HBPC clinic with responsibility to provide direct services for adult and geriatric Veterans and their families.
Total Rewards of a Allied Health Professional Duties of the Senior Social Worker include, but are not limited to: Conducts psychosocial assessments to determine needs of Veterans and family members Active participant in the treatment planning process with other disciplines Participates in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate Provides individual, family and group counseling as appropriate Provides consultation/education to Veterans and families on community resources, advance directives and VA benefits Provide consultation/education to social work interns on social work practice and VA policy and procedure Provides wellness/prevention education and facilitate Patient/family support groups as appropriate Maintains data and statistical compilations to comply with Joint Commission, VA and medical center policy and procedures Assists patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death Conducts training for staff, students and community providers regarding HBPC and will provide technical assistance and educational sessions to community organizations as needed Needs to be able to operate a government vehicle, so a valid state driver's license is required. If transporting Veteran patients, must take mandatory annual physical and driver training Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
