You will lead discharge planning, collaborate with physicians, nurses, and therapists, manage a caseload with attention to Social Determinants of Health, and contribute to performance improvement and denials support.

Qualificações

• Master of Social Work required.
• Experience in acute or post-acute social work and case management.
• Ability to coordinate discharge planning and community resources.

Responsabilidades

• Interpret, analyze, and apply psychosocial data to determine care plans.
• Develop and implement discharge plans for admitted patients.
• Co-manage caseload with RN Case Managers.
• Identify social determinants of health and link to resources.
• Communicate with physicians, nurses, and insurers to support post-acute care.
• Enter data in data collection systems per policy.
