Job summary

Apploi is seeking a Social Worker to oversee the Social Services Department within a facility, focusing on residents’ highest practicable well-being and the provision of medically-related social services in line with state and federal regulations.

You will complete comprehensive assessments, contribute to individualized care plans, and encourage resident and family involvement in planning. The role requires documenting actions, attending meetings, and upholding confidentiality.

Responsibilities

• Oversee development, implementation, supervision and evaluation of Social Services to meet residents’ wellbeing.
• Identify need for medically-related social services and ensure compliance with state/federal regulations.
• Complete social services component of comprehensive assessments for assigned residents.
• Contribute to care plans with individualized goals and non-pharmacological approaches.
• Encourage resident/family participation in care plan meetings and review Advance Directives as needed.
• Attend care plan, QAPI, and other committee meetings; complete required trainings and licensing requirements.
• Document all social work actions in residents’ medical records, maintain confidentiality, and report noncompliance or abuse as required.
• Assist with staff education on Resident Rights and dignity; support policy development and annual reviews.
