Job summary

Apploi is seeking a Social Services Director to lead and manage the social services team at our nursing home, ensuring resident well-being and high-quality support. You will oversee programs, collaborate with residents, families and clinical staff to develop individualized care plans, and provide counseling during transitions while maintaining regulatory compliance.

Qualifications

• Bachelor’s degree in Social Work or related field (Master’s preferred).
• Social Worker license in New Jersey.
• Experience in long-term care or healthcare settings is a plus.
• Strong leadership, communication, and organizational skills.
• Familiarity with federal and state regulations for nursing homes.

Responsibilities

• Oversee and coordinate social services programs to support residents’ emotional and social well-being.
• Collaborate with residents, families, and healthcare staff to develop individualized care plans.
• Provide counseling and support to residents and families during transitions or challenges.
• Ensure compliance with state and federal regulations related to social services in long-term care.
• Lead a team of social workers and maintain accurate documentation.
