Qualifications

• Master’s Degree in Social Work or a human services field is required.
• CSW or LSW credential in the state of operation is required.
• 3+ years of Social Work experience are required; long term care/skilled nursing experience preferred.

Responsibilities

• Plan and direct a comprehensive social service program for residents and families.
• Participate in interdisciplinary team meetings to support the Center population.
• Provide therapeutic intervention for residents’ social and psychological needs.
