Qualifications

• Bachelor's degree in Social Work or Human Services (or equivalent experience).
• Licensure: Valid state certification or licensure as required to practice social work in long-term care.

Responsibilities

• Oversee the operation of the nursing center Social Services Department per NASW Code of Ethics.
• Foster a climate that maximizes patients' independence, dignity, and well-being.
• Directly supervise two or more Social Services staff.
• Ensure psychosocial assessments identify needs and strengths for each patient/resident.
• Assist with discharge goals and planning at admission.
• Educate staff on the psychosocial needs of patients and families.
