Qualifications

Responsibilities

• Complete and document screens and assessments; respond to identified needs.
• Develop and deliver daily therapeutic program; provide group and individual therapy.
• Lead discharge planning starting on admission and coordinate with community resources.
• Serve as contact for interfacility probate processes and collaboration with community agencies.
• Support trauma-informed, person-centered care within the therapeutic milieu.
