Outreach Carol Stream is seeking a Part-Time Older Adults Program Director (with clinical licensure) to serve as the clinical lead for senior services. You will provide direct clinical care, crisis intervention, and care coordination for older adults while overseeing program quality, supervision, and partnerships.

The role supports ethical practice, risk management, and development of comprehensive, clinically grounded services for seniors within a faith-based mission-driven community.

Qualifications

• Master’s degree and clinical license in social work or related field (LCSW or LCPC) required.
• Previous experience working with older adults; sensitivity to their needs and navigating related systems.
• Sensitivity to the needs of a low-income, culturally diverse population.
• Previous experience in nursing or medical field is a plus.
• Valid Illinois driver's license, reliable transportation, and safe driving record.
• Ability to lift, move and carry event supplies (up to 25lbs).
• Commitment to Outreach's Statement of Mission & Faith.

Responsabilités

• Provide clinical assessment, intervention, and case management services to older adults.
• Conduct mental health and wellness assessments and develop service plans.
• Deliver crisis intervention, short-term counseling, and ongoing clinical monitoring.
• Provide advocacy and care coordination with referrals to medical, behavioral health, housing, and community resources.
• Maintain timely, ethical clinical documentation in line with standards.
• Provide clinical leadership and supervision of the Friendly Visiting Program.
• Plan and evaluate a diverse year-round calendar of programs for healthy aging and engagement.
• Manage program operations including schedules, transportation, volunteers, budgets and facilities.
• Promote participant engagement through outreach and evaluation of outcomes.
• Screen and assess volunteers with training in trauma-informed and elder-sensitive care.
• Supervise and mentor interns (BSW/MSW), with clinical guidance and evaluations.
• Collaborate with community agencies to strengthen referral pathways.
• Contribute to program evaluation and senior services model development with leadership.
