Job summary

Welby Health is seeking a Lead Care Manager to supervise a dedicated team of care managers and medical assistants. You will oversee a remote patient monitoring and chronic care management patient panel, ensure documentation accuracy, and drive high-quality clinical outcomes for multiple practices.

The role requires a registered nurse with leadership experience, strong data literacy, and the ability to translate clinical strategy into executable team actions.

Qualifications

• Bachelor’s degree in nursing or related healthcare field.
• Active and valid RN license in good standing.
• 3–5 years of clinical experience in care management or population health.
• Experience in a lead or supervisory clinical role with direct reports.

Responsibilities

• Lead a defined remote patient monitoring or chronic care management panel.
• Direct the daily performance and documentation standards of the team.
• Coach team members and escalate gaps in collaboration or capacity.
• Monitor team KPIs and intervene when at-risk trends appear.
• Represent Welby Health in client performance reviews and escalation calls.
• Ensure compliance with regulatory requirements and billing guidelines.
