Job summary

Rocky Mountain Home Health and Hospice - Utah seeks a Registered Nurse to direct daily nursing care for residents and oversee Certified Nursing Assistants in a long-term care setting. You will administer medications, review physician orders, and ensure compliance with state and federal regulations.

This exempt, full-time role requires CPR certification, an active Utah RN license, and aligns with the facility’s care plans, discharge planning, and interdisciplinary collaboration.

Qualifications

• Nursing degree from an accredited college or university.
• Current unrestricted RN license in practicing state.
• Current CPR certification.
• Prior long-term care or assisted living experience preferred but not required.

Responsibilities

• Directs the daily activities of the certified nursing assistants and caregivers in accordance with regulations and policies.
• Administers medications as per physicians’ orders and delegates duties as allowed by guidelines.
• Participates in admission, discharge and transfers of residents and plans care accordingly.
• Assesses changes in residents’ status, communicates with physicians and families, and documents appropriately.
• Prepares care plans, coordinates with the interdisciplinary team, and ensures compliance with infection control practices.
• Supports end-of-life care when required and participates in QA/QI activities.
• Maintains medication stock, documents physician orders, and ensures appropriate follow-up care.
