Qualifications

• RN licensure in Connecticut, in good standing.
• MSN + 1 year in home health or BSN + 3 years clinical experience (1 year in home health) or ADN/Diploma + 3 years clinical experience (1 year in home health) with community health training.

Responsibilities

• Oversee and support the delivery of direct care services.
• Manage new referrals and admission assessments.
• Monitor and revise individualized care plans.
• Provide supervision and performance evaluations for nursing staff.
• Contribute to development of clinical standards.
• Participate in hiring and training of clinical personnel.
• Ensure communication with clients, families, and staff.
