Qualifications

• RN license current in the state where practice will occur.
• Proof of CPR is required.
• Valid driver's license, auto insurance and reliable transportation.
• Experience in home health settings.
• At least 2 years as RN with 1+ year in a management role in home care/hospice.

Responsibilities

• Oversee clinical operations for the location and ensure quality patient care onsite.
• Review referrals, admissions, and care plans to meet standards.
• Guide clinicians, support goal setting, and extend clinical decision support.
• Ensure documentation, audits, and billing meet Medicare and payer standards.
• Participate in hiring, training, performance reviews, coaching, and actions.
• Provide ongoing staff education based on QA data and reviews.
• Coordinate communication among physicians, team and caregivers for care coordination.
• Contribute to quality improvement, data tracking, budgeting and community relations.
• Provide direct patient care in exceptional circumstances and act as Branch Director when needed.
