Job summary

Humana Inc. is seeking a Clinical Manager for CenterWell Home Health in Indiana. You will lead a team of clinicians delivering home-based care, guide clinical practice, coordinate patient services, and ensure high-quality care while promoting staff development.

This on-site role requires RN licensure, CPR, valid driver's license, and management experience in home health or a similar setting; involvement in hiring, education, and quality improvement with branch leadership.

Qualifications

• Must hold a current RN license in the state.
• Proof of CPR certification is required.
• Valid driver's license with reliable transportation is necessary.
• Minimum of 2 years as an RN with at least 1 year in a management role in home health or equivalent.

Responsibilities

• Oversee clinical operations for the location, onsite in-office.
• Review referrals, determine admission appropriateness, assign clinicians, ensure Plans of Care meet standards.
• Guide and educate clinicians; support care planning and clinical decision-making.
• Ensure documentation, audits, and billing meet Medicare and payer standards; monitor outcomes.
• Participate in hiring, training, performance evaluation, and coaching of staff.
• Coordinate communication among physicians, team members and caregivers to support care coordination.
• Contribute to quality improvement, budgeting, marketing and community relationships.
