Job summary

CenterWell Home Health, part of Humana Inc., is seeking a Clinical Manager RN to lead a team of clinicians delivering care in patients' homes. You will oversee clinical operations, coordinate referrals, and ensure Plans of Care meet standards while guiding practice and supporting staff.

This on-site role requires RN license, CPR, a valid driver's license, and at least two years as an RN with one year in management within home health or hospice.

Qualifications

• Current state license as a Registered Nurse.
• Proof of current CPR.
• Valid driver's license, auto insurance and reliable transportation.
• Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment.

Responsibilities

• Oversee clinical operations for the location, including patient care delivery, staff management, documentation quality, and regulatory compliance, onsite.
• Review referrals, determine admission appropriateness, assign clinicians, and ensure Plans of Care meet patient needs and agency standards.
• Guide, support, and educate clinicians; assist with goal‑setting, care planning, and clinical decisions; provide clinical support during operating hours.
• Ensure clinical documentation, audits, and billing meet Medicare, payer and company standards; monitor case management quality.
• Participate in hiring, training, performance evaluation, coaching and corrective actions for clinical staff.
• Coordinate communication among physicians, team members and caregivers for care coordination and discharge planning.
