Job summary

Qualifications

• Graduate of an accredited BSN with RN license.
• ADN with minimum 2 years nursing experience, including 12 months in the area of concentration.

Responsibilities

• Demonstrates advanced assessment skills and plans care to improve patient outcomes.
• Acts as a resource to nurses and collaborates with the interdisciplinary team.
• Documents the nursing process accurately and suggests changes to those processes.
• Plans and implements patient care, modifying as needed with input from patient/family.
• Evaluates and alters the plan of care promptly when necessary.
• Identifies patient/family learning needs and connects them with resources.
• Uses the AACN Synergy model in clinical practice.
• Works within leadership roles and mentors staff when assigned.
