Qualifications

• Must be a graduate of a state-approved school of professional registered nursing.
• RN license in the state(s) of practice, current and unrestricted.
• BSN preferred.
• Two years of nursing experience including one year in home care.
• One year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management for assigned clients.
• Supervise and coordinate RN Case Managers under regional leadership.
• Ensure adherence to nursing scope of practice and agency policy.
• Plan, implement, and evaluate care; lead client care conferences as needed.
• Collaborate with staffing to address scheduling and competencies.
• Support performance improvement and accurate clinical documentation.
