Job summary

Cnscares in Lenoir City, TN, is seeking an experienced RN Case Manager for a full-time, Monday-Friday role with on-call rotation. You will oversee coordinated care for a patient group and supervise related clinical activities in-home.

The ideal candidate holds an RN license with an associate or bachelor’s degree, 3 years RN experience, and at least 1 year in case management. Benefits include medical/dental/vision, 401K, HSA, unlimited PTO, and mileage reimbursement.

Qualifications

RN license is required.
3 years of experience as a Registered Nurse.
At least 1 year of case management experience.
Home health experience preferred.

Responsibilities

Provide accurate documentation with rationale to support reauthorization of patients.
Deliver in-home RN visits and attend physician appointments 50% of the time.
Assist in implementing and amending the Plan of Care as needed.
Conduct orientation and onboarding of new staff for patients’ homes.
Serve as a resource to train and support new staff after training.
Submit nursing documentation timely per policy.
Perform chart reviews within established timeframes and address documentation issues.
Promote collaboration with patients, physicians, payers and the healthcare team.
Assist authorization team to communicate information to payers for reauthorization.
Contribute to process improvements for clinical, financial and satisfaction outcomes.
Participate in quality assurance committees and interdepartmental projects.
Stay current with payor policy and procedures.
Assist with state survey prep and act as backup administrator.
Perform other duties as assigned.
