This full-time, hybrid role requires RN licensure in Washington, 5 years of clinical nursing experience, and supervisory or care management experience.

Bachelor's degree preferred.

Qualifications

• RN licensure in Washington is required.
• 5 years of clinical nursing experience in acute care or ambulatory settings.
• Supervisory experience in case management or care coordination preferred.

Responsibilities

• Provide leadership and supervision of daily clinical and administrative operations of the care management team.
• Supervise care management staff including nurses, social workers, and care coordinators to meet organizational goals.
• Collaborate with inpatient and ambulatory clinicians, PacMed and PCN leaders to support cost-effective, high-quality care.
