Job summary

Responsibilities

• Complete assessments for residents to develop individualized care and rehab plans.
• Develop and update care and rehab plans to support safe recovery and optimal outcomes.
• Administer medications and treatments per physician orders and rehab protocol.
• Coordinate with therapy and interdisciplinary teams to ensure cohesive care.
• Educate residents and families about care plans and progress.
• Document accurately in the EMR and maintain up-to-date records.
• Support CNAs and other staff to deliver high-quality patient care.
