Job summary

Legacy Healthcare Services, Inc. is seeking a Rehab Director to lead the rehabilitation program at a skilled nursing facility.

You will promote Legacy policies, supervise therapy teams, and coordinate with facility staff to deliver high-quality care to aging adults. You will oversee clinical programs, manage staff schedules, and contribute to outcomes reporting while aligning with state and federal standards.

Qualifications

• Clinical background in rehabilitation therapies (PT/OT/SLP) or related.
• 3–5 years of experience in long-term care or senior living.
• Licensed and registered in state where assigned.

Responsibilities

• Oversee site productivity and manage budget-related changes.
• Maintain a caseload to support rehab department standards.
• Assess patient needs and develop appropriate clinical programs.
• Direct and train staff in clinical service delivery across disciplines.
• Coordinate daily rehab department activities with facility staff.
• Participate in clinical outcomes reporting and share results with the customer.
• Manage staff schedules to meet caseload requirements and use scheduling tools.
• Ensure physician signatures on therapy plans of cares and maintain documentation.
