SUNY Downstate Health Sciences University in Brooklyn seeks a full-time Utilization Review & Quality Assurance Senior Coordinator / Rehabilitation Case Manager to lead case reviews and coordinate discharge planning within the Rehabilitation Medicine team.

The role requires a BSN or MD, NY licensure, 2+ years in case management, and strong communication and organizational skills; experience with care management platforms is preferred. On-site day shift with collaborative team Rounds.

Formación

• Bachelor of Science Degree in Nursing (BSN) or Medical Doctor Degree (MD).
• New York State Registered Nurse Licensure or Medical Doctor Licensure.
• 2+ years of experience in case management.
• Computer proficiency in Microsoft Word, Excel, PowerPoint.
• Strong interpersonal, communication, administrative, and organizational skills.
• Or, a satisfactory equivalent combination of experience, education and training to the above.

Responsabilidades

• Review patient records for medical necessity for admission to IRF per MCGs.
• Provide feedback per MCGs and collaborate with social workers, case managers and physicians.
• Validate admission and continuing stay criteria with payers and physicians.
• Complete clinical reviews and forward to MCOs; assess patient progression toward outcomes.
• Develop and evaluate discharge plans with the multidisciplinary team.
• Communicate with patients and care teams to address reimbursement and discharge options.
