Qualifications

• Licensed Clinical Social Worker (LCSW) required.
• Certified Case Manager (CCM) required.
• Master's degree in Social Work required.
• Leadership experience in a healthcare setting preferred.

Responsibilities

• Oversee administrative management for the rehab case management department, including clinical, operating, and financial outcomes.
• Collaborate with patients, families, volunteers, administration, medical staff, peers, and staff to achieve departmental goals.
• Partner with Rehab leadership on strategic planning, program development, and system-wide service delivery integration.
