Job summary

Heights Care & Rehabilitation in Denver, CO is seeking a caring Physical Therapy Assistant to join our In House Team. You will work in a skilled nursing environment, assisted living and outpatient therapy, helping seniors regain quality of life and independence.

Pay is based on experience; you will collaborate with Licensed Physical Therapists to deliver high-quality care and document patient progress in line with facility and regulatory requirements.

Qualifications

• Licensed Physical Therapy Assistant in good standing.
• Ability to work with seniors in SNF, assisted living and outpatient settings.
• Strong communication of patient progress with the therapy team.

Responsibilities

• Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical Therapist.
• Communicate pertinent information about patient progress and plans to the Physical Therapist.
• Record treatment notes and contribute to discharge summaries per policy.
