Qualifications

• Active Alabama RN license required.

Responsibilities

• Assess patients on admission and ongoing, including physical, psychological, social, spiritual, cultural, nutritional and functional needs.
• Plan and document patient and family education based on learning barriers.
• Coordinate care planning with patient, family, providers and disciplines to achieve outcomes.
• Assign and delegate nursing care to team members according to patient needs and team capabilities.
• Document observations, care provided and changes in the care plan.
• Provide handoffs and participate in unit-based huddles.
• Support PI activities and implement improvements as needed.
• Perform competency assessments for each job class.
