Job summary

Search Station is seeking a licensed Physical Therapist for a full-time, on-site role in Modesto, CA. You will evaluate, treat, and document patient care within a skilled nursing facility setting, collaborating with an interdisciplinary rehab team.

The role emphasizes quality patient care, billing accuracy, and compliance, with a schedule of 8:00AM–5:00PM, Monday–Friday, and a compensation range of $60–$70 per hour.

Qualifications

• Graduate of an approved Physical Therapy curriculum.
• Active PT license without encumbrances; CPR as required for license renewal.
• Knowledge of Medicare and payer sources; resident rights awareness.
• Long-term care experience preferred; PDPM knowledge preferred.

Responsibilities

• Initiate evaluations per physician orders and payer guidelines.
• Develop and provide treatment plans aligning with benefits and reimbursement levels.
• Educate staff and third-party payers on treatment benefits and functional outcomes.
• Complete timely documentation including evaluations, progress notes, and monthly reports.
• Enter key information into EMR and rehab systems; supervise assistants and students.
• Meet productivity standards and participate in interdisciplinary co-treatment.
