Job summary

Search Station is recruiting a Physical Therapist for a confidential long-term care facility in Merced, CA. You will evaluate, treat, plan, educate families, and document care within a rehab team.

Requirements include a PT degree, active license, and knowledge of Medicare and LTC regulations. Prior LTC and PDPM familiarity are preferred; strong multitasking and supervision of rehab staff are expected.

Qualifications

• Graduate of an approved Physical Therapy curriculum.
• Current, active PT license — able to practice unencumbered.
• Knowledge of Medicare and other payer sources.
• Knowledge of resident rights.
• Long-term care experience preferred.
• PDPM knowledge preferred.
• Knowledge of Group Therapy & Sensory Therapy preferred.
• Ability to manage multiple tasks simultaneously.

Responsibilities

• Initiate evaluations per physician orders and payer authorization/guidelines.
• Develop and provide treatment plans aligned with benefit and reimbursement levels.
• Educate staff and payers on treatment benefits and outcomes.
• Complete timely documentation including evaluations and progress notes.
• Enter information into EMR and rehab systems.
• Attend continuing education and in-service trainings.
