Job summary

Conexio Care seeks a Psychiatric Prescriber to lead a multidisciplinary Assertive Community Treatment (ACT) team, providing psychiatric assessment, diagnosis, treatment, and ongoing care for clients with severe mental illness and co‑occurring challenges in Delaware.

The role emphasizes person-centered recovery planning, trauma‑informed practice, and coordination with hospitals and community providers, including crisis response and documentation in the electronic health record.

Qualifications

• MD or DO degree from an accredited medical school.
• Successful completion of an approved psychiatry residency program.
• Board Certified or Board Eligible in Psychiatry by a recognized certifying body.
• Current unrestricted license to practice medicine in the State of Delaware.
• Active DEA registration.

Responsibilities

• Provide clinical leadership and guidance to the multidisciplinary ACT Team.
• Collaborate in development and revision of recovery plans.
• Conduct comprehensive psychiatric evaluations and establish treatment recommendations.
• Prescribe, monitor, and adjust psychotropic medications; provide medication reviews.
• Deliver face-to-face psychiatric services in office, community, hospital, and home-based settings.
• Coordinate psychiatric care with primary care and hospitals; ensure continuous care.
