Qualifications

• Bachelor's degree in social work, psychology, or related human services field with two years of mental health or intellectual disabilities experience.
• One year of experience as a Psychiatric Caseworker I in Nevada State Service or equivalent
• Interview clients for psychosocial rehabilitation program placement and develop service/care coordination plans
• Develop treatment plans and participate in multidisciplinary team meetings and coordinating services

Responsibilities

• Perform intake and assessment; prepare care plans and monitor progress.
• Coordinate services among multidisciplinary teams and community resources.
• Attend court and report on client status and needs.
• Provide crisis intervention and wraparound case management.
• Develop discharge plans and housing referrals.
• Document progress and communicate with supervisors.
