Job summary

Qualifications

• Bachelor's degree in social work, psychology, or closely related human services field required.
• Two years of experience in a mental health or intellectual disabilities setting, with one year of professional casework in psychosocial rehabilitation.
• Or one year of experience as Psychiatric Caseworker I in Nevada State Service or equivalent.
• Must be able to interview clients, develop service/care coordination plans, and document progress.

Responsibilities

• Develop and monitor plans of care for youth with complex needs.
• Coordinate with community services and families to implement service plans.
• Attend multidisciplinary team meetings to set goals and adjust strategies.
• Provide ongoing individual and family management and education on daily living, coping, and employment.
• Discharge planning and housing referrals as part of care coordination.
• Maintain case records, documentation, and required progress notes.
