Job summary

CenterWell Senior Primary Care, part of Humana, seeks a Primary Care Physician to provide comprehensive outpatient care to adults and seniors in a collaborative, patient-centered environment at the Surprise, AZ clinic.

You will join a multidisciplinary team, maintain thorough medical records, and contribute to quality improvement initiatives while focusing on improving outcomes for geriatric and adult populations.

Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• TB screening required if selected for the role.

Responsibilities

• Deliver outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement projects.
• Work with interdisciplinary teams to ensure holistic care.
• Contribute to projects and innovations in care delivery.

Education
